Lucky Palliative Services, Inc. / Palliative Care provides residential care in patients' homes and has established partnerships with long-term care facilities and nursing homes, assisted living facilities and residential care facilities for the elderly.
(Intensive Comfort Care SM): If a patient develops the need for more constant care, we can provide shifts of hospice staff at the home for a brief period so the patient can avoid being transferred to a hospital.
Services, Inc. / Palliative Care has special arrangements with local hospitals and other facilities if patients require additional support that can't be provided at home. Patients may receive necessary and appropriate care and return home as soon as possible.
Limited to five consecutive days, respite care provides a period of rest for the patient's primary caregiver. Home care patients will be admitted to an in-patient facility without having to meet the normal pain and symptom management criteria.
A serious advancing life-limiting illness can be a difficult condition to confront-let alone to talk about with your loved ones and friends. Yet, for many of us, the day will come when we need to confront limitations resulting from serious illness and consider how we would like to be treated if out condition became terminal. How might we like our life's final chapters to unfold? The Lucky Palliative Services, Inc. / Palliative Care team advocate for the rights of terminally ill patients and their families. We believe that patients should be apprised of their clinical prognosis and be involved in the decision-making process that determines how their end-of-life care is provided.
